Blue Cross/Blue Shield HMO
This health maintenance organization is a referral system designed to provide employees with access to health care routed from within the Blue Cross/Blue Shield network. Under the HMO plan, employees are required to select a primary care physician (PCP).
Blue Cross/Blue Shield PPO
The preferred provider organization plan is designed to provide employees with open access to any independent physician, hospital, or other health-care provider who participates in the Blue Cross/Blue Shield network. Members may also choose to receive services from providers outside of the network covered at a lower, "non-preferred" level after meeting the deductible.
Dental
The DeltaPremier plan is a managed dental-care plan designed to offer employees a broad range of preventative, restorative and orthodontic services.
Fitness Center Reimbursement
Egenera recognizes the importance of regular exercise, and will match the Blue Cross/Blue Shield reimbursement of $150 per year for employees who join a fitness center.
Short Term
The short-term disability plan offered by Egenera provides up to 67 percent of an employee's wages, from the first day if from an accident or the eighth day if from an illness, for up to 13 weeks, if medically necessary.
Long Term
The long-term disability plan offered by Egenera pays up to 60 percent of an employee's wages after 90 days of disability.
Life
Life insurance is provided by the Company. Employees are automatically covered for twice their basic annual earnings with a guaranteed issue maximum.
Supplemental Life
Employees may elect to purchase additional insurance for themselves or their spouses and children. Employee options are for once, twice and thrice basic annual earnings.
A Flexible Spending Account is available which allows employees to contribute on a pre-tax basis into accounts for medical or dependant-care expenses.
A 401K plan is available for employees to contribute on a pre-tax basis into a retirement account. A menu of 26 Fidelity funds is provided as investment options.
Egenera provides employee assistance through Horizon Care - a free, voluntary, confidential counseling service which provides professional help to employees and their family members.
Egenera recognizes that all employees need to recharge! The Company sponsors eight paid holidays, two paid floating holidays and three weeks' paid vacation per year.
